Healthchecks

All registered patients get a yearly health check with the Southern Independent Medical Practice.

We will also send you a Healthcheck booking reminder when your next health check is due – nice and easy for you!

How to book

You can choose which medical service and/or vaccination you are interested in by selecting your choice from 'Our Services' or by the individual Health Plan.

Once you have chosen the medical service/s you require, just click 'Request Call Back'.

Enter your contact details and click on 'Submit' (note the services you previously selected are included in your call back request to us).

Your call back request will be sent to us and we will contact you as soon as we are able.
